PERMISSION TO USE IMAGES AND VIDEO
I Hereby grant permission for Calvary Chapel Hernando to record sounds, images, of video of my child listed above while attending the VBS
program. I also give permission to Calvary Chapel Hernando at its sole discretion, to use these sounds, images , or videos in publications (including print, websites, and social media platforms) owned by Calvary Chapel Hernando in relation to this VBS program.
